Who qualifies for accutane is decided in a consultation rather than by a checklist, though there are recognizable criteria underneath the conversation. Severity matters, history matters, and safety considerations can settle the question regardless of how bad the acne looks.

Who Qualifies for Accutane on Severity Alone?

Patients with severe nodular acne, and patients whose acne is visibly scarring. Those two groups are the clearest cases and they rarely need a long discussion. Deep lesions that heal into permanent marks make the argument on their own.

Severity is judged on examination rather than on photographs from your phone, though bringing photographs of your worst weeks genuinely helps.

Who Qualifies for Accutane After Other Treatments?

Patients whose acne is moderate but has outlasted a proper attempt at everything else. That is where the detail of your history becomes decisive, because a treatment used for six weeks and abandoned is not the same as one used properly for months.

Bring details of
Every antibiotic course, with rough dates and duration
Topical treatments, and how consistently they were used
Hormonal treatments if you have tried them
What improved, what did nothing, and what you stopped
Any reaction that made you abandon a treatment early

What Rules Someone Out?

Pregnancy, or planning one in the near future, settles it immediately. Certain liver conditions, significantly abnormal blood fats, and some medicines taken alongside it can also make a course unwise. An allergy to any component rules it out outright.

Several of these are temporary rather than permanent. A medicine can be changed and a blood result can improve, so being told not yet is not the same as being told never.

Mental health history is asked about too, and it is worth saying plainly that it does not usually rule anyone out. What it changes is how closely mood is watched and who else stays in the loop. Patients who conceal it in case they are refused end up with the one version of the course that genuinely is less safe.

Does Age Decide It?

Not on its own. It is prescribed across a wide age range, from teenagers through to adults well past the years people associate with acne. What age does affect is the surrounding conversation, since consent, contraception and monitoring look different at different stages of life.
