The strongest signal is scarring that is actively forming. Redness fades on its own, but a depression in the skin does not, and every month of untreated nodular acne can add more. When that is happening, cautious waiting is not the safe option it appears to be.

An examination, a full treatment history, photographs, and a frank conversation about what the course demands. Then enrollment, baseline bloodwork, and the contraception requirements if they apply to you. Nothing is dispensed until all of that is in place.
Expect to be asked about mood and about pregnancy plans directly, in more detail than feels comfortable at the time, because those two questions carry more weight in this particular decision than anything else on the form does.
The appointment is longer than a routine one. Set aside proper time for it, and bring a list, because the volume of information handed over in that single visit is genuinely hard to absorb in one sitting.

Then say so, and take the time. A dermatologist who cannot answer your questions without pressure is not one to proceed with. It is entirely reasonable to have the consultation, go away, and come back in a few weeks having thought about it.
