Modafinil and Anxiety: When It Eases and When It Makes Things Worse

Modafinil can increase anxiety, nervousness and difficulty settling at night, particularly early on and particularly at higher doses. It can also appear to reduce anxiety in people whose anxiety was driven by the strain of being exhausted all day.

Both are real, and which one you get depends more on what was underneath the sleepiness than on the medicine itself.

More arousal can feel like more anxiety. That is usually dose-related.

Why Can Modafinil Anxiety Get Sharper at First?

A wake-promoting medicine raises arousal. For someone already running at a high baseline, more arousal feels like more anxiety. A tight chest, a busier mind, an inability to sit still. Caffeine on top of it makes the same effect larger.

This tends to show early, and it tends to be dose-related, which is why the answer is often a smaller dose rather than no medicine at all.

Why Does It Sometimes Seem to Ease?

Chronic sleepiness generates its own anxiety: the fear of falling asleep at the wheel, of being caught out at work, of not being able to rely on yourself. When the sleepiness lifts, that layer often lifts with it, and people describe feeling calmer.

That is a genuine improvement, but it is the treatment of the sleep disorder doing the work, not an anti-anxiety effect of the drug.

What Should You Watch For?

  • Anxiety that is new, or noticeably worse than your usual baseline.
  • Agitation, irritability, or feeling wound up in a way you cannot settle.
  • Difficulty falling asleep that started with the medicine.
  • Low mood, hopelessness, or thoughts of harming yourself, report these promptly, not at the next review.
  • Unusual suspiciousness, racing thoughts, or perceptions that others do not share, seek care straight away.

Modafinil anxiety when a disorder is already diagnosed

Having anxiety does not automatically rule out modafinil, but it changes the conversation. Tell your prescriber what you take for it, because interactions matter here, and agree in advance what would count as a signal to stop. Deciding that when you feel well is much easier than deciding it in a bad week.

What to report without waiting

Contact your prescriber promptly if you develop a rash of any kind, chest pain, a fast or irregular heartbeat, breathlessness, or new agitation, low mood or thoughts of harming yourself. A rash means stopping modafinil and seeking medical advice the same day rather than waiting for your next appointment.
